I have an extra Pert Plus/Sure School Supplies Rebate form to give away! I got my rebate today, which was sooner than I expected. You have to buy $5 worth of Pert Plus or Sure products AND $5 of school supplies together, send in the rebate form and receipt, with the UPC codes of the Pert Plus/Sure products AND the school supplies on the back of the receipt.
